PLEASE NOTE: With patient access to eye care services now increasing in Ontario, we are no longer accepting requests from the public for e-Assessments. If you are a non-Prism patient who requires care, please see your optometrist or family doctor.
If you do not have an optometrist, you can find one in your area on the College of Optometrists of Ontario Website.

FOR PRISM PATIENTS WITH A SCHEDULED APPOINTMENT
If you have a confirmed in-person appointment at Prism, you MUST check-in online 4 to 7 days before attending your appointment. Instructions for attending your visit are HERE and on the online check-in form. Instructions will also be emailed to you when you submit the form.
FOR PRISM PATIENTS WHO WANT TO CONTACT US
If you are a Prism patient who wished to contact us with questions or concerns about your eye health, you can contact us using our secure online form. You can also contact our call centre at (905) 456-3937. Due to high call volumes, we recommend the online form for your convenience.
FOR DOCTORS REFERRING PATIENTS
Please use our secure online portal to refer your patients. You can send referrals with or without an account. It is easy to use, completely secure, allows you to track your patients progress, and utilizes digital communications to update your patients on your behalf.
