If you are a NEW PRISM PATIENT (I have not been to Prism before or I have a new referral) and have a confirmed in-person appointment at Prism, you MUST check-in online 4 to 7 days before attending your appointment. Instructions for attending your visit are HERE and on the online check-in form. Instructions will also be emailed to you when you submit the form.
